avifile for gcc 4.2(.2) fixed

good ev'ning to everyone,

I just fixed avifile to work with gcc-4.2(.2)
There were two missing unsigned's in 

(samples/qtvidcap)/kv4lsetup.cpp
and
\1/v4lxif.cpp

gcc refuses to work without those since 4.2 
or at least 4.2.2 and since avifile does not seem
to be maintained atm, i made two small patches(attached).
